Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. In reach in cooler in kitchen area, two packs of ground beef on top of case of fish. Operator moved ground beef to lower shelf.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. At front counter in steam table, collard greens (101F - Hot Holding). Owner had product double pan because it was getting too hot for approximately 30 minutes. Owner removed second pan and rechecked at 168F.
Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Ceiling tiles in hallway and kitchen area not smooth and easily cleanable.

At front lobby warming unit, cooked fish (92F - Hot Holding); cooked beef with vegetables (110F - Hot Holding). Both items placed in warming unit for less than three hours. Manager had employee reheat food above 165F.
