Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Automatic vacuum breaker missing from added splitter attachment at mop sink.
No proof of required state approved employee training provided for employee hired more than 60 days ago. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Operator unable to provide proof of any training for food handling employees employed over 60 days. Per operator, manager has training certificates, but manager is not currently present.
In-use utensil used with moist food stored in running water with insufficient velocity to flush food particles away. Upon inspection, dipperwell water was turned off while utensils were sitting in standing water. Operator turned dipperwell back on during inspection.
Stored food not covered. Italian ice stored in reach-in freezers in back of kitchen in igloo drink coolers with no lid.
