Observed employee eating bread by the preparation table at kitchen then continued to prepare food without washing her hands prior. Coached employee on proper hand washing techniques.
Nonfood-grade bags used in direct contact with food. Observed in the double glass door cooler at prep area pizza dough stored in direct-contact with thank you bags. Explained to operator that for direct food contact he needs to get food graded bags.

Dead roaches on premises. Observed 2 dead roaches on the floor behind a file cabinet at the office space located in the kitchen. Operator removed cabinet and cleaned the floor removing the two roaches on site.
