Toxic substance/chemical improperly stored. Quaternary tablets on shelf next to seasoning and single use gloves and above prep sink. Employee removed tablets and stored with other chemicals.
No paper towels or mechanical hand drying device provided at handwash sink. No paper towels available in ladies restroom. Operator filled paper towels.
Bowl or other container with no handle used to dispense food. In container of sugar on bottom shelf of storage rack next bag- in- box soda rack has to go container used as scoop. Employee removed container from sugar.
Dead roaches on premises. Observed 1 dead roach on floor in back kitchen under bread proofer. Operator cleaned up dead roach and discarded.
Floor, wall and/or ceiling soiled/has accumulation of debris. Floor under prep sink soiled with food debris. Also, floor under bag- in- box soda rack soiled with food debris. Also, floor under bread proofer soiled with food debris. Also, floor under grill at front counter and to go bagging table at front counter soiled with food debris. Also, wall behind prep sink soiled with mold-like substance. Also, wall behind bag- in- box rack soiled with dried residue.
Garbage on the ground and/or pad around dumpster. On ground around shared dumpster out back of establishment have trash. Regional manager stated he has been in contact with property manager multiple times to have issue resolved.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Plumbing pipe under prep sink soiled with white slime like substance. Also, walk-in cooler fan cover has small amount of dust build up. On soda machine in lobby around nozzles soiled with debris. Also, drip tray of soda machine in lobby soiled with mold-like substance. Also, hood filters above grill soiled.
Reach-in cooler interior/shelves have accumulation of soil residues. Interior edge under flip top lid and interior of cooler at front counter soiled with food debris. Also, bottom portion of glass display case at front counter soiled with food debris.
Unwashed fruits/vegetables stored with ready-to-eat food. On back storage rack next bag- in- box soda rack unwashed tomatoes stored above bag- in- box soda boxes.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. At front counter, cut tomatoes (50F - Cold Holding). Employee stated they were placed at front counter from walk-in cooler about 1 hour prior. Tomatoes were used up on customer food right after temperature taken.
Handwash sink not accessible for employee use at all times. Handwash sink across from trip sink had case of tomatoes on top of sink when arrived at establishment. Manager moved case of tomatoes off the handwash sink.
Spray bottle containing toxic substance not labeled. Hanging on rack above produce sink, spray bottle of clear liquid not labeled. Manager labeled spray bottle.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. On prep table next to triple sink, employee open to go cup next to pan of cut tomatoes and slicer. Manager discarded drink cup into trash.
Employee eating in a food preparation or other restricted area. On prep table next to triple sink, employees half eaten sandwich on table next to pan of cut tomatoes and slicers. Manager discarded food into trash.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. On prep table next to triple sink, employee cell phone on table playing music next to slicer and pan of cut tomatoes. Manager removed cell phone from table.

Unwashed fruits/vegetables stored with ready-to-eat food. In walk-in cooler, unwashed celery and green peppers stored on top shelf above case of sliced cheese and pan of prepped to go containers of sauteed mushrooms.

No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Establishment ran out of chemical test strips, per manager. Inspector tested using own strips.
No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. No proof of any employee trainings. Manager stated he just took over location and is in process of getting employee training complete.
