Food-contact surface soiled with food debris, mold-like substance or slime. Observed not in use meat slicer with lettuce debris, meat slicer with meat debris, sandwich station with bread and lettuce debris and food prep table with bread debris ( all not in use ).
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed a laptop, phone and a speaker stored on food contact table next to pan of tomatoes and a knife and 3 cut loaves of bread. Manager removed items from table.
Floor not cleaned when the least amount of food is exposed. Observed floors soiled with excessive heavy amounts of food debris and paper debris in kitchen behind counter and under and behind equipment. Employee began sweeping floor after pointing out debris.
Food stored on floor. Observed a Case of turkey breast and case of red and green peppers stored on floor in walk in cooler at 4:15 today. Manager stated truck was received at 12:10 today.

Stored food not covered. Food stored in Glass display cooler- turkey breast 43F, ham 43F, roast beef 42F, block cheese 43F, chicken salad 46F, tuna salad 47F (3:00-4:15) not covered.
Wet mop not stored in a manner to allow the mop to dry. Observed mop stored in standing water in mop bucket near mop sink.

No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Inspector provided compliance form to manager, she went over responsibilities and had them sign form.
Employee personal food not properly identified and segregated from food to be served to the public. Employee Cole slaw stored on shelf in walk in cooler next to food for consumers.
Food stored on floor. Case of tomatoes supported on floor between back exit door and walk in freezer.
Ice buildup in reach-in freezer and/or walk-in freezer. Build up of ice in walk in freezer on floor.
In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Scoop handle stored in corn meal. Employee removed scoop from corn meal and sent to warehouse washing area.
Wet wiping cloth not stored in sanitizing solution between uses. 3 wet soiled cloths hanging from handwashing sink and prep sink not stored in sanitizer solution.
