Employee used the bathroom and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands upon returning to employee area. Observed employee come back to cook line and then proceeded to change gloves without washing hands and then proceeded to touch napkins to place in customers bag.

Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee clean meat sliced with wiping cloth and then proceeded to handle ready to eat bread without changing gloves and washing hands. Discussed with operator. Employee washed hands and changed gloves.
Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Observed in walk in cooler tuna fish 63F at 1:45 to 61F at 2:05 (cooling since 1:00). At this current rate of cooling tuna fish will not reach 41F or below within four hours. Observed cooling covered. Observed cooling in large container. Operator removed cover and placed into shallow container.
