Observed employee grab raw chicken philly meat and put on grill then change gloves and grab clean utensils without first washing hands. Observed second employee put on gloves and begin to prepare customer order without first washing hands.
Employee touched face/hair and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee scratch face with hand and continue to prepare customer order without changing gloves and washing hands.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed sliced tomatoes (46F - Cold Holding) at front counter, as per operator tomatoes were placed at front counter 10 mins prior to inspection.

Employee was coached on appropriate hygiene practices. Following the discussion, the employee washed their hands as instructed
