Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Operator emptied sink and refilled with fresh solution at 200 ppm. Triple Sink (Quaternary 100ppm)
Food-contact surface soiled with food debris, mold-like substance or slime. Manual tomato slicer unclean. Observed tomato pieces in the blade blocks.
Handwash sink not accessible for employee use at all times. Hand sink by compartment sink was blocked by a bread rack and a stepstool.
Carbon dioxide/helium tanks not adequately secured.. Operator chained tanks during the visit.

Operator moved these items into walk-in cooler for quick cooling. Corrective action taken. Also on front line display, cut tomatoes (57F - Cold Holding); cut lettuce (50F - Cold Holding) for 2 hours per operator. Provided operator with time as a public health control form.
Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Observed tomatoes in reach-in cooler covered, stacked densely.
Handwash sink not accessible for employee use at all times in back of kitchen. Operator removed rack blocking sink.
Carbon dioxide/helium tanks not adequately secured. Unsecured tanks in back of kitchen. Operator secured the tanks.
