Server handled soiled dishes or utensils and then picked up plated food, served food, or prepared a beverage without washing hands. Employee wearing glovestook soiled tray to dishwash area and returned to sandwich prep area and handled bread. Also took another soiled container to dishwash area then removed gloves and put on clean gloves but did not wash hands before putting on gloves. Spoke to Manager and she discussed with employee
Dead roaches on premises. One dead roach in cabinet under self serve tea dispenser in dining area. Manager removed

No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. For one female employee on site, employee filled out the agreement during the inspection.
No written procedures available for use of time as a public health control to hold time/temperature control for safety food. No documents available, also no time marked on front counter products (lettuce and cut tomatoes). Inspector provided a digital copy.
Spray bottle containing toxic substance not labeled. At the front counter area, spray bottle with blue liquid not labeled, manage labeled sanitizer.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. At the dining area, soda base soiled, also walk-in cooler shelves soiled.
