Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Observed at 0ppm. Operator replaced empty sanitizer bottle, retested at 200ppm.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed at mop sink.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed food buildup on self serve soda fountain.
Working containers of food removed from original container not identified by common name. Observed bulk sugar not labeled. Manager placed label on container.

Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Observed 3 compartment sink sanitizer at 100ppm. Checked new fill of sanitizer bucket from automated system, 300ppm. Operator discard the sanitizer solution and re-sanitized dishes in the sanitizer solution and dishes on the drain board. Recommended to manager to check sanitizer strength more frequently.
