A return visit. The problems from the previous inspection had been dealt with.
Ordered shut on the spot. 3 violations recorded, 1 of them the tier the state says can make you ill. A reinspection is required before this kitchen may reopen.

Raw sewage on ground of establishment. Observed : raw sewage and paper products emitting from an in ground clean out behind the establishment. The back door and ramp are not in the current sewage field. There is a blue shed that is used by this license for old equipment, spare parts and 5T-shirts. The shed is currently not in the sewage field, but the raw sewage blocks access to the shed. The assistant manger relates there is nothing in the shed that they access on a regular basis. 2 Plumbers have been requested, none are on sight. When the toilet is flushed, the water flow increases from the clean out.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed : the right hand Coke nozzle in the dining area is soiled. ALSO, the veggie dicer has dried food debris on it, while on the clean dish shelf.
Working containers of food removed from original container not identified by common name. Observed : the bulk sugar lacks a label.

Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed : front line staff touch visor then apron and continue food production. They stopped and washed hands and changed gloves. The manager followed up and and discussed it with the staff member.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed: Observed lack of back flow device on the non chemical side of the splitter in the mop sink. The atmospheric hub is corroded and possibly constantly leaking. The vacuum breaker on the spout has taken its place. Add a device to the NON- chemical side of the splitter.

Food-contact surface soiled with food debris, mold-like substance or slime. Observed the 'grey Y' ice activator at the soda machine has an accumulation of foreign substance. The staff worked on cleaning it. ALSO, the tomato slicer blade and sliding plates had dried food debris on them. This was placed in hot soapy water to soak.
