Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee grabbed a tray from outside walk in freezer, then continue working in pizza station with same gloves, after manager educated employee, gloves discarded, employee washed hands and put in new pair of gloves.
Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. Two doors reach in freezer- Raw chicken bags over open uncovered tray of meat balls. Employee moved chicken to bottom shelf.
