Observed chlorine sanitizer dishwasher machine at 0ppm. While conducting inspect technician fix the machine, rested at 50 ppm.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed an opener blade soiled at preparation table.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed over entrance of walk-in cooler.
