Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee using mop bucket then put on gloves and use clean utensils without washing hands. Discussed proper hand washing practices with Operator. Employee washed hands during inspection prior to putting on gloves.
Screen in door torn/in poor repair. Observed screen window in back of kitchen with a tear across from mop sink.
