Raw animal food stored over or with unwashed produce. Observed: raw eggs stored over the unwashed jalapeños in the reach-in cooler.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed: in the upright residential reach-in cooler, Bulk cheese 44°, Ambient air temperature eggs 46°. This unit had the freezer door propped open. ALSO, in the flip top, shredded cheese, mozzarella, 45°. This unit is across from the flat top and grill area, 91° Ambient air temperature on the unit.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed: the ceiling of the microwave has an accumulation of food debris. ALSO, the meat cutting board is stained.

Buildup of food debris/soil residue on equipment door handles. Observed: the reach-in freezer door handle is soiled.
Employee personal food not properly identified and segregated from food to be served to the public.... Observed: employee chicken stored over guest food in the reach-in cooler. This was not marked EMP.
Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Observed the 3 compartment sink is labeled: Rinse Wash Sanitize.
In-use wet wiping cloth/towel used under cutting board. Observed: a damp cloth under the meat cutting board.
Mobile food dispensing vehicle license number not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle. Observed: lack of exterior license number 5251054 in minimum 2 inch numbers on the service side of the unit.
No Heimlich maneuver/choking sign posted. Observed: the choking poster is in the 3 ring binder, and is not posted.

No conspicuously located ambient air temperature thermometer in holding unit. Reach in cooler and reach in freezer had thermometers that read from 100F and higher.
