Single-use gloves not changed as needed after changing tasks or when damaged or soiled. -Observed chef cracks raw shell eggs and then touch sandwich bread without changing gloves and wash hands. After brief education, employee then change her gloves and wash hands.
Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. -Observed at 10:27 a.m. no time mark ont TCS cheeese at cookline held using time as a public health control. Per operator, cheese placed at cookline at 10:15 a.m. operator correctly time mark cheese.
Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas.
Food stored in a location that is exposed to splash/dust. -Observed bags with food stored next to hand wash sink in wait station. Operator removed and stored properly.

Chemical test kit not used to ensure proper sanitization of equipment and utensils when using a chemical sanitizer. No chlorine test strips available for dish machine.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. No QT test strips for quaternary sanitizer used in three compartment sink and wiping cloths.
No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. Handwash sink not accessible for employee use at all times. Observed hand wash sink at cook line blocked by stool. Operator removed.
Insect control device installed over food preparation area. Observed bug zapper installed over dry storage shelf in dish wash area.
