Open box of raw frozen hamburger patties stored on top of boxes of frozen vegetables and corn on the cob inside reach in freezer. Operator relocated raw burger patties to an approved location during time of inspection.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee used her cellphone, then put on gloves to handled packaged food and clean food equipments. Discussed with manager, employee washed hands and replaced gloves at time of inspection.
Food-contact surfaces not sanitized after cleaning, before use. Do not use equipment/utensils not properly sanitized. Observed employee washed and rinsed the food container, then placed on drain shelf to air dry without sanitized. Discussed with manager, manager moved the container back to three compartment sink.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. A container of cut lettuce (57F - Cold Holding) in reach-in cooler. Operator stated that it was left on prep table for about 30 minutes, and moved to reach-in freezer for rapid cooling. Re temped: cut lettuce 40F.
