Rodent activity present as evidenced by rodent droppings found. Observed; Approximately 50 rodent droppings on not-in-use reach in cooler by oven on main cook line and on canned foods items on top of reach in cooler. Approximately 30 rodent droppings on floor around not-in-use reach in cooler next to oven on main cook line. Approximately 20 rodent droppings on sheet tray container various oil and sauces on bottom shelf of main prep table by reach in cooler on main cook line. Approximately 200 rodent droppings in storage area near electrical box in bakery section. Approximately 50 rodent droppings on floor and shelving of dry storage area with spices nearby in bakery area. Approximately 150 rodent droppings on floor under single service item shelving unit and not-in-use reach in cooler in bakery area. Approximately 30 rodent droppings on floor around door way of walk-in cooler and shelving units inside walk-in cooler.

Observed establishment conducting reduced oxygen packaging with no submitted or approved HACCP plan. Cooked food items in packaged reduced oxygen environments held more than 48 hours after packaging date.

Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed sanitizer bucket with quatanery ammonium solution testing at 500+ppm. Person in charge voluntarily discarded and replaced with solution testing at 200ppm.
Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed opened container of half and half with no date marker employee stated was opened 2 days prior.

Observed dish machine in cooking kitchen with chlorine testing at 0ppm. Person in charge called for maintenance and set up three compartment sink.

In walk in cooler next to prep area of kitchen, grits (45F - Cold Holding); rice (45F - Cold Holding); cooked green beans (45F - Cold Holding); cooked Brussels sprouts (45F - Cold Holding). Walk in cooler possibly in defrost mode, compressor kicked on while taking temps, operator dropped temps turn. All items 43F or below.
Toxic substance/chemical improperly stored. Bottle of Clorox bleach located next to chocolate chips and flour. Operator removed chemicals.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. On outside triple sink, no vacuum breaker.
Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Operator did not have clean up plan. Supplied operator with form DBPR HR 5030-104.
Cardboard used to line food-contact shelves. In dry storage area of kitchen, cardboard placed under sauce bottles. Operator removed the cardboard.
