Operator says he didn't change anything, contacted plan review and he will need to submit updated plans.

Employee switched from working with raw food to ready-to-eat food without washing hands. Employee touching raw Hamburg with gloved hands then continued to handle bag of buns and other clean equipment without washing hands and changing gloves between tasks.
Nonfood-grade bags used in direct contact with food. Multiple bags of raw chicken and raw beef in grocery bags throughout cools and freezer.
Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. Raw chicken, removed from original packaging, stored over boxes of French fries in walk in freezer. Also raw shell eggs stored over cheese in walk in cooler. Raw chicken and raw beef stored over raw pork in reach in freezer under service window, all items removed from original packaging.
