No paper towels or mechanical hand drying device provided at handwash sink at women bathroom. Staff provided a new roll of towels.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed one new employee without signed health reporting agreement, as per manager hired less than two weeks. Manager provided employee with health reporting agreement document, employee read it and signed it.
No soap provided at handwash sink. Observed HWS with not soap next to 3CS in kitchen area. Staff provided new soap refill.
Packaged food not labeled as specified by law. Observed tomatoes soup and pumpkin soup proved by a third party stored in boxes not labeled specified by law.
