Ceiling/ceiling tile shows damage or is in disrepair. Observed damaged ceiling tile in back of dinning room near kitchen where a leak did happen but has since been fixed. Tile has been painted black in color and shows white spots due to water ruining black paint. Manager showed me the new tiles and plans to replace the damaged one today after they close.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed 0PPM from dishwasher machine. Manager stated Ecolab would be coming in today to fix machine. Set up 3 compartment sink to wash dishes in in the meantime.
Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands. Observed cook on cook line crack raw eggs with gloves on then touch a food container without washing hands or changing gloves.
Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw eggs stored above containers of shredded cheese in pull top reach in cooler in back of cookline next to stove. Manager moved eggs to other side of cooler.
Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed female busser empty bus tub using gloved hands, then run the gloved hands under water and towel dried. No glove change. Spoke with manager about proper handwashing techniques.
Chemical test kit not used to ensure proper sanitization of equipment and utensils when using a chemical sanitizer.
Food-contact surface soiled with food debris and hard staining inside cut groove marks.
No paper towels or mechanical hand drying device provided at handwash sink. Observed at sink on cook line and in dish area. Operator replaced paper towels.
Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Micheal expired. Operator stated his is signed up to retrain.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed on flap guard in large ice machine on wall next to handwashing sink. Discussed with manager cleaning the flap more often.
