Employee failed to wash hands before putting on gloves to initiate a task working with food. Food worker observed putting on gloves without washing hands first.. discussed with manager.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. 4 way splitter installed, 1 of the 4 goes to the chemical system, the other 3 require vacuum breakers to be installed.
Wiping cloth and three compartment sink sanitizer solution exceeds the maximum concentration allowed. 500 or more on quatinary sanitizer, operator diluted it and will continue to dilute it until dispenser can be adjusted.
Water with a temperature of at least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Hand wash sink by drive thru 80F,
