Observed the following food temperatures greater than 41°F; shredded lettuce (72F - Cold Holding); cut tomatoes (70F - Cold Holding); sliced cheese (56F - Cold Holding). All were located on front counter prep table. As per manager food had only been out for about 1 hour. Manager moved to reach in cooler for cooling.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee cups, keys and phone stored on prep table. Manager removed.
Food stored on floor. Observed bag of onions and oil being stored on floor in prep area. Manager moved to shelf.
