Raw animal food stored over/not properly separated from ready-to-eat food. Raw beef stored over cooked gyro meat in reach in cooler. Also, raw fish stored over pies in reach in freezer. Also, raw shell eggs stored over ready-to-eat sauces in flip top reach in cooler. Operator rearranged all products to proper storage.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Outside back exit, threaded faucet with attached hose is missing a vacuum breaker.
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Wiping cloth solution in kitchen tested at 200+ppm chlorine. Operator remade solution to 100ppm chlorine.

Chlorine sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Sanitizer solution in three compartment sink tested at 10ppm chlorine. Operator adjusted solution to 50ppm chlorine.
Non-food grade paper/paper towel used as liner for food container. Paper towels used as food liner in blueberries and parsley. Operator removed paper towels.
Cardboard used to line nonfood-contact shelves. Cardboard used as liner for grill dome lids. Operator replaced cardboard with plastic tray liner.

Gyro meat in a broiler that is turned off. Gyro broiler was turned off. Per operator, after slicing, unit was turned off for approximately 10 minutes. Operator turned unit back on.

Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. See stop sale. Observed individually packaged, sealed and completely thawed salmon in reach in cooler in the kitchen. Label on salmon states to remove from packaging before thawing.
Outer openings not protected with self-closing doors. Self-closing device on back exit door is broken.
