Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee taking an order touching the cashier, and proceeded to put gloves on and proceeded to scoop ice cream. Manager coach employee to wash hands before putting gloves.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed splitter connected no vacuum breaker by mop sink.
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed Sanitizer Bucket (Chlorine 200ppm) by front counter. Manager prepared new solution. Recheck Sanitizer Bucket (Chlorine 50ppm).
Handwash sink used for purposes other than handwashing. Observed a sponge stored inside hand sink front counter. Manager removed.
