Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed the cook go outside and return to the kitchen and put on gloves to prepare an order without washing his hands first.
Garbage can located outside has no lid or lid open/broken.
Wet wiping cloths used for occasional spills on equipment food- and nonfood-contact surfaces not clean. The chef put the towel in the sanitizer bucket.
