Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. At bar, Triple Sink (Quaternary 0ppm). Operator stated he will make a new sanitizer sink.
Raw animal food stored over/not properly separated from ready-to-eat food. In stand up white cooler near dish area, metal pan of raw egg yokes above ready to eat sauce portions and pies. Operator moved egg yokes. Also, in stand up reach in cooler in back kitchen, pan of raw beef and pan of raw scallops over cooked wings. Operator moved wings to higher shelf.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In reach in cooler at front cook line, raw beef (45F - Cold Holding); raw chicken (46F - Cold Holding); shredded cheese (50F - Cold Holding). All items put in cooler less then 3 hours but doors are not sealed tightly.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In steam table in back kitchen, rice (110F - Hot Holding, retemped at 162F); grits (126F - Hot Holding, retemped at 167). Operator stated items been in warmer for approximately 3 hours and reheated items.
Handwash sink used for purposes other than handwashing. At bar, handwash sink has plastic cup and food debris in drain. Operator removed cup and cleaned out sink.

Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Cook on cook line handled bread with bare hands. Cook stopped, washed hands, and put on gloves to re-engage in food preparation.
