Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed raw pork (52F - Cold Holding); cooked chicken (52F - Cold Holding); cooked beef (52F - Cold Holding); raw beef (52F - Cold Holding); raw pork (52F - Cold Holding); cooked chicken (51F - Cold Holding) in the walk-in cooler. As per the manager he took temperatures at approximately 11:00 this morning. The manager moved all TCS foods to the reach in cooler.
Handwash sink not accessible for employee use at all times. Observed a rack in front of the hand washing sink next to the walk-in cooler. The manager moved it.

As per the manager the food has been on line for approximately two and a half hours. The manager removed the food and placed in the walk-in cooler for a quick chill.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed the can opener blade soiled.
Handwash sink not accessible for employee use at all times. Observed a plastic container in the sink. The cook removed it.
