Displayed food not properly protected from contamination. At the front area, raw vegetables and fruits inside wired baskets with no overhead protection, Manager moved some of the products to the back and placed a display only sign.
Label on a food item prepared and packaged onsite for customer self-service does not include product identity/description, date product was packaged and name and address of the establishment that prepared and packaged the food. At the front counter area (grab and go), desserts and peanut butter bites no address provided. Manager moved all item behind counter.
