1. Raw animal food stored over/not properly separated from ready-to-eat food. Raw chicken stored over salads and cooked arepas in flip top cooler. Operator removed raw chicken to bottom shelf. 2. Observed raw ground beef stored over containers of soups in walk in cooler. Operator removed ground beef to safe location. 3. Observed raw fish over cooked empanadas. Operator removed raw fish and placed in a safe location.
Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Observed manager came from outside began touching food items before washing hands. Advised manager to wash her hands in wish she did.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed at 10:43am sausage and empanadas at 109F at front hot holding unit. As per operator both items transferee to unit at 8:00 am for hot holding. Operator reheated items sausage (198F and empanadas to 185F.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.
Equipment drain line draining into handwash sink. Observed ice machine drain line draining into frontline hand washing sink. Operator removed drain from sink.
