Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee touched the bread slice with his bare hand. Bread was discarded, employee washed his hands and put gloves on his hands.
Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed one of the hand sink in the kitchen with not hot running water. Person in charge opened the hot line faucet immediately.
Bowl or other container with no handle used to dispense food. Observed plastic container with no handle used to scoop oatmeal.Person in charge discarded it.
In-use ice scoop stored on soiled surface between uses. Observed scoop on the top of the ice machine. Person in charge sent it to ware washing area immediately.

In-use utensil not stored on a clean portion of food preparation or cooking equipment. Observed the steel stored in between reach in coolers at the main cook line. The utensil was removed and sent to the warewashing area.
In-use utensil stored in sanitizer between uses. Observed a knife held inside sanitizing solution. Sous chef discarded the solution and rinsed the knife.

In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed ice cream scoop inside standing water. Person in charge discarded the water.
