Equipment in poor repair. Observed walk in freezer door in need of repair with door panel chipped. Also observed in poor condition door gasket of walk in freezer and gasket of reach in coolers across near cook line.
Exterior door has a gap at the threshold that opens to the outside. Observed a hole mid way up the exit door located near the bathrooms.
Floor tiles missing and/or in disrepair. Observed cracked tiles through out ware washing and kitchen areas.
No waste receptacle installed at handwash sink provided with disposable towels. Observed for hand wash station at kitchen entrance.

Warewashing sanitizing solution exceeding the maximum concentration allowed. Observed Dishwasher (Chlorine +200ppm). Operator called the technician to come to review the concentration.
