Operator stated items went in cooler this morning but lid was left open. Operator closed lid. Operator went back and removed both items and placed in bottom of reach-in cooler to chill.

Establishment advertised a specific fish on the menu/menu board but served another type of fish. Establishment advertising tilapia on menu and when inquired with cook and operator both directed me to swap that is served as tilapia.
