Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Employee touching washed/peeled mango with bare hands while peeling before cold holding until served. Operator discussed with employee who then put on gloves.
In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Scoop handle laying in coconut and nuts inside storage containers. Operator removed scoops from containers.

Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. At flip top cooler; cut melon (64F - Ambient Cooling 1:45pm/64F 2:10pm). Operator stated melon cut 2 hours prior, stored in bowls above make top, at this rate will not reach 41F within 4 hours, moved to reach in cooler to facilitate faster cooling.
Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. At hand wash sink; water not draining. Operator removed stopper from sink, water draining.
