In front counter reach in cooler, cooked onions (70F - Cooling @ 7 hours). Operator stated onions were done at 8:10 am and put in 1 gallon plastic container with lid and put in bottom of cooler. Temperature taken at 3 pm. See stop sale.
Stop Sale issued on time/temperature control for safety food due to temperature abuse. In front counter reach in cooler, cooked onions (70F - Cooling @ 7 hours). Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In top of flip top reach in cooler on front counter, shell less hard boiled egg (57F - Cold Holding). Eggs are in pan with water with lid open right in front of the exit side of toaster oven. Eggs were stocked no more than 1 hour ago. Operator put eggs in bottom of reach in cooler to bring temperature down.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of ice machine soiled with white/black mold-like substance. Operator began to clean.

Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee Gatorade bottle on top shelf of metal rack behind registers above single service items. Operator moved employee drink. Also, employee mug on back prep table next to single service items and tomatoes. Operator had employee move cup.
Equipment and utensils not properly air-dried - wet nesting. On clean dish rack, multiple pans stacked before properly air drying.
Equipment in poor repair. Multiple gaskets on reach in coolers in establishment torn. Also, flip top reach in cooler at front counter has duct tape on lid/handle.
Floor, wall and/or ceiling soiled/has accumulation of debris. On ceiling vents at either end of front counter soiled with dust build up. Also, lower wall under 3 compartment sink soiled with residue.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Interior of microwave at front counter soiled with food debris.
Open dumpster lid. Dumpster lid open. Operator will send employee out to close it.
Unwrapped single-service utensils not presented so that only the handles are touched. On soda counter in lobby, plastic knife stored for customers self service not all have handles facing out first.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In reach in cooler on back line, cut lettuce (53F - Cold Holding). Cut lettuce placed in top of reach in cooler in pan still in bag. Manager stated bag was placed in cooler approximately 2 hours ago. Manager removed bag from top of reach in cooler and put in bottom section to lower temperature.
Toxic substance/chemical improperly stored. In men's restroom in storage cabinet, toilet bowl cleaner and stainless steel cleaner stored next to toilet paper. Manager moved chemicals away from other products.
