Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 70 degrees Fahrenheit within two hours and from 135 degrees Fahrenheit to 41 degrees Fahrenheit within six hours. See stop sale. Observed cooked potatoes started at 1400 (76F - Cooling) at 1710 see stop sale
Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed chlorine dish machine with an out of order sign. Person in charge does not understand how to hook up chemicals on this style machine. Person in charge has triple sink set up.
Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Observed cook dip hands in bleach water bucket and grab cooking utensils to start cooking food. Immediately stopped and discussed hand washing with employee. Employee washed hands in hand washing station.
Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed cook plate ready to eat cheesy bread with bare hands. No visible contamination observed on bread. Discussed with person in charge.
Nonfood-grade containers used for food storage - direct contact with food. Observed bread stored in reused liquor boxes without the use of bags or liners. Person in charge moved bread into metal food container during inspection. No observed contamination on bread.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed egg wash on flip-top cooler (77F - Cold Holding) left out for 10 min per person in charge. Person in charge placed in cooler too cool.

In reach-in cooler across from cook line, pasta (48F - Cold Holding); pasta (50F - Cold Holding); raw salmon (48F - Cold Holding); calamari (45F - Cold Holding) Operator stated pasta had been in cooler since the day before and discarded. Operator put salmon and calamari in different cooler to cool down. Operator stated he had been in and out of the cooler multiple times.
