Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. On cook line steam well: cooked chicken (109F - Hot Holding); cooked rice (119F - Hot Holding); cooked seasoned rice (124F - Hot Holding); cooked beef (111F - Hot Holding) for less than one hour per operator. Operator returned items to grill for rapid reheating, cooked chicken reached 174F within 5 minutes, cooked rice and seasoned rice to 167 within 15 minutes, cooked beef to 166F within 20 minutes.
Accumulation of black/green mold-like substance in the chute of the ice machine/bin right side customer self service soda machine.

Observed employee washing dirty dishes, then wrapped meat for storage in walk-in cooler no handwash. Discussed with employee proper handwashing, employee washed hands before returning to work.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Two way splitter installed, missing one vacuum breaker on right side.

Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. In walk-in cooler observed approximately 25# cut tomatoes ambient cooling in large covered plastic container, approximately six inches deep. Provided operator with cooling handout and discussed ambient cooling techniques with operator.
