The operator has been working with Cassie from Plan Review on the items she replaced after taking over. Plan Review has her current plans, and the operator will send updated plans showing the revised walkway area to the hand sink.

12 dead flies were found inside the refrigerator on the right side of the kitchen near the reach-in freezer. 10 dead flies were found inside the refrigerator by the 3-compartment sink. The operator cleaned up the dead flies and sanitized the area.
Dead roaches on premises. 1 dead roach found by the table near the door. The operator swept up the dead road and sanitize the area.
To the left of the exit door, the floor is covered with a liquid slime. The operator cleaned up the liquid slime.
A return visit. The problems from the previous inspection had been dealt with.
A return visit while the kitchen was already shut. The problems had not been fixed, so the closure stayed in place. 1 violations recorded, 1 high priority.

Ordered shut on the spot. 7 violations recorded, 3 of them the tier the state says can make you ill. A reinspection is required before this kitchen may reopen.

Presence of 18 live ant like insects present by shelf near soda can storage area. Operator sprayed, eliminated, cleaned, and sanitized.
Rodent activity present as evidenced by rodent droppings found. 42 rodent droppings underneath three compartment sink, behind equipment, and on top of storage containers.
Handwash sink not accessible for employee use at all times and blocked by cart with press. Operator corrected.
No soap provided at handwash sink to the right of the three compartment sink. Operator placed soap.
Well water testing report/documentation is not available upon request.
Ceiling/ceiling tile shows damage or is in disrepair in kitchen above electric panel area in closet to the right of the handwash sink and to the right of the rear exit door.
