Cove molding at floor/wall juncture broken/missing. Cove molding in disrepair on right wall of walk-in cooler. Cove molding missing behind floor mixer. Multiple floor tiles missing under the three compartment sink.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee backpack, beverages, food and cellphone stored on top of cases of pizza sauce by back exit. Manger moved all items to proper storage.
Floor/wall/ceiling/ceiling vent soiled/has accumulation of debris. Ceiling vents in kitchen and prep area have dust and black mold-like substance. Wall at pizza oven is soiled. Wall above shelving at back prep area has dust.

Also, ceiling tiles around vents/vents in lobby and kitchen area soiled with dust. Also, fan above drive through window has dust build up.
In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. In reach in cooler on cook line, handle of scoop in shredded parm cheese. Manager removed handle of scoop from product and above rim of container.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Bottom tray of pizza oven soiled with food debris build up.
No handwashing sign provided at a hand sink used by food employees. No handwash sign provided in unisex bathroom in lobby used by employees and customers. Provided handwashing sign to operate via email during inspection that he printed out and displayed in bathroom.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets on reach in cooler on cook line and pizza holding units at front counter soiled with food debris. Also, plastic shelves in back hallways and to right of pizza oven used for clean pizza trays soiled with food debris. Also, interior of self service pizza warmer soiled with food debris. Also, pizza holding rack at end of cook line and storage racks in walk-in cooler soiled.
