Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Set up three-compartment sink QUAT 0PPM. Inspector had employee drain and refill sink, then 200PPM. Employee resanitized pans washed in sanitizer previously.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Four employees working in kitchen who have not signed the employee health reporting agreement. Inspector provided management with a copy during this inspection.
