Observed two boxes of cheese pizza with no time mark, operator stated they were made at 10:30 am and discarded both pizzas stating she did not realize the stamp was not working when they were printed out.
Toxic substance/chemical improperly stored. Observed equipment cleaner in a spray bottle on a prep table next to a reach in freezer and a spray bottle hanging on a prep table next to the ovens, operator removed.
Handwash sink not accessible for employee use at all times. Observed hand washing sink in front of kitchen had equipment blocking making it not accessible, employee moved the equipment.
Manager or person in charge lacking proof of food manager certification and no other certified food service manager employed at this location.
Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Observed the dispenser is not working and unable to get paper towels out in the front restroom in lobby area.
