Observed approximately 30 live insects flying all around the mop sink. Observed 3+ live insects flying around the three compartment sink in the kitchen. Observed around 2 live insects flying at front counter area by the office area located in the kitchen. Also observed 1 live maggot crawling on the floor by exit door.
Presence of insects, rodents, or other pests. Observed approximately 35+ dead maggots on the floor inside the kitchen by the exit door.
Handwash sink used for purposes other than handwashing. Observed employee washing a whisk and scissor in the hand wash sink located at preparation area. Also observed employee filling up container with water in the hand washing sink at preparation area.
