Toxic substance/chemical improperly stored. Orange chemical in a spray bottle was sitting directly adjacent to bags of baking soda, used for food recipes. Operator moved bottle away from food. Pink chemical in a spray bottle (contained bleach) observed sitting on a lower prep shelf directly adjacent to jugs of crazy bread sauce and wing sauce. Operator moved bottle away from food. Orange spray cleaner chemical observed sitting directly adjacent to clean pizza pans, stored as clean, on an upper storage rack. Operator moved bottle away from clean pans. Repeat from 12/14/2022

Handwash sink not accessible for employee use at all times. Back handwash sink was blocked by a large Hobart mixer siting directly in front and a bottle Lipton Tea in the basin.

No soap provided at handwash sink. This was the only accessible handwash sink in the restaurant. Advised Operator to move soap from the blocked sink to the sink that was accessible.
A minimum of one bathroom facility is not available for public use. The restroom in the customer area is locked with a sign stating "for employee use only"
Employee personal food not properly identified and segregated from food to be served to the public. Employee drink and opened candy bar was sitting on top of a rack that contained pizza dough in the walk-in cooler.
Food not stored at least 6 inches off of the floor. Bucket of sauce attached to the sauce machine (Wunder-bar) was sitting directly on the floor.
Food-contact surface not smooth and easily cleanable. Large, table- mounted can opener blade observed to be severely rusted.
