Observed one employee clean floor near front line cook and handle phone for orders, then proceed to wear gloves without first washing hands. Discussed with employee to wash hands after every change of task and before handling food. Employee washed hands and wore new gloves at time of inspection.
Toxic substance/chemical improperly stored. One bottle of sanitizer spray bottle and aerosol spray can stored on top of reach in beverage cooler located behind front line service station. Operator relocated sanitizer spray and aerosol spray to an approved location at time of inspection.
Vacuum breaker missing at splitter added to mop sink faucet. Located at the exterior back of establishment adjacent to walk-in cooler for silver hose.
Food-contact surface soiled with food debris, mold-like substance or slime. Multiple cutting boards throughout establishment are soiled. Interior of ice dispenser soiled with black mold like substance, located in soda fountain on front line cook.
