Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed the dishwasher machine testing at around 10ppm. Discussed setting up three compartment sink. Tested sink at 150ppm.

Handwash sink used for purposes other than handwashing. Handwashing sinks behind bar had strainers and dishes within them. Manager took out dishes and strainers. We discussed only using hand washing sinks for hand washing.
Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Miso salmon served medium but has no indication of it being served undercooked on menu. Manager placed asterisks next to consumer advisory then placed asterisks next to salmon on menu.
Cutting board has cut marks and is no longer cleanable. Long white cutting boards on cookline have cut marks that are no longer cleanable.
