Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Fish, fish, octopus in prep area make line top portion of unit. Person in charge states had unit open. Closed lid and moved to reach in freezer. In walk in cooler pasta, beef, beef 44-45F. Person in charge states have been in and out of unit, had delivery 1 hour ago. Person in charge turned unit down during inspection, will monitor.

Bathroom located inside establishment not completely enclosed with tight-fitting, self-closing doors. Restrooms in establishment missing self closing doors.
Ceiling/ceiling tile shows damage or is in disrepair. Observed leak at newly enclosed area in front of walk in cooler and at door frame to new storage area. Also floors in ew storage area appear to be unsealed concrete. Also, hole observed around pipe near side door to back kitchen area. Hole observed around wiring in back newly enclosed area by two reach in freezers. Also, tape used to hold ac vent in kitchen area.
Dead roaches on premises. Observed one dead roach in new dry storage area. Observed one dead roach in employee restroom.
Fly sticky tape hanging over food/food preparation area/food-contact equipment. Flying insect sticky trap observed over steam well/ prep area. Person in charge removed.
Ice buildup in reach-in freezer and/or walk-in freezer. Small reach in freezer at bar area with ice accumulation.
