Operator filled cold well with ice. 2nd temp: Pico de Gallo 39 F, diced tomato 44 F, shredded lettuce 42 F, shredded white cheese 45F.

Time/temperature control for safety food reheated for hot holding not reaching 165 degrees Fahrenheit for 15 seconds within 2 hours. Black beans reheated in steam well at 120°, corrected action taken, inspector had operator reheated black beans to 165° and place in steam to hold at 135°, black beans 177°.
Handwash sink not accessible for employee use at all times. Both employee hand wash sinks blocked by fan and bucket, corrected, manager moved items during inspection.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths.
