Dishwasher (Chlorine 0ppm). Sanitizer container empty. Operator changed container and primed. Retest at 50ppm.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed cook place raw chicken on the grill, remove gloves and went to put on new gloves. Inspector notified employee of washing hands in between glove changes. Employee washed hands and put on new gloves.
Clean linens stored on floor. Bag of clean wiping cloths on the floor by back office. Employee moved off the floor.
Unclean building components, attachments or fixtures. Fan guards in walk in cooler are dusty. Floor in walk in freezer is soiled.
