Observed at reach in cooler PORK BELLY (55F - Cold Holding) as per operator more than four hours.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employees wearing bracelets while engaging in food preparation. Manager had employees remove jewelry.

As per employee, less than 1 hour. Coached manager on correct procedure. Product was moved to cooler for rapid cooling.
No paper towels or mechanical hand drying device provided at handwash sink. Observed at hand washing sink next to three compartment sink. Paper towels were provided.
Cutting board has cut marks and is no longer cleanable. Observed both white cutting boards at kitchen area grooved.

Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Observed and tested quaternary sanitizer bucket at 0ppm. Employee filled a new sanitizer bucket with quaternary sanitizer, test again at 400ppm.
