Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee went from cleaning prep table with cloth, put on gloves and then began preparing food without washing hands. Operator discussed with employee who then washed hands and changed gloves.
Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. At reach in freezer; storage container raw fish roe stored over portioned ready to eat wasabi. Operator moved raw fish roe to bottom shelf.
Vacuum breaker missing at splitter added to mop sink faucet. Advised Operator to install vacuum breaker on splitter with hose attached.
